It is imperative to have a comprehensive document that is evidence based and supports care provision. It should clearly record the care provided, and have prompts to alert professionals to risk factors according to the NICE guidelines.

Prompts include:

1. Traffic light diagram on page 1 listing important information for women planning a pregnancy.

2. Prompts for healthcare professionals to discuss various issues: diet and exercise, smoking cessation, sick day rules, hypoglycaemia.

3. Space to document a managment plan to outline specific treatment and care agreed between the woman and care providers.

4. Space to document individual consultations.
